It is located near the Leprosarium, but is one of the few locations that does not have a proper Vestige that players can travel to. How To Get To the Tower of Penance In Lords of the Fallen

To get to the Tower of Penance, players must make their way to the Vestige of Brother Jeremiah, which is located in Hallowed Brothers Leprosarium. After resting at the Vestige, head outside the building and enter the building opposite you. The door to the building might be locked at first, and you will have to deal with the enemy that is roaming here, who drops the key. Once inside the building, head up the stairs ahead and turn left.

Turn right at the end and head up the stairs on the right side. Turn left up the stairs, then left again, and once you reach the end, turn right. You will see a bridge up ahead—roll through the wooden blockade and head inside the building up ahead. Head straight through the doorway and turn right. If the door on the right is closed, then you will have to go down the stairs and take the long way around.

Inside the building, take the exit on the right to get outside to the courtyard with the trees. Ignore the Crusaders in the middle and head through the wooden doorway. You will find some cultists on the side that you can ignore and head through the doorway up ahead.

From a distance, you will be able to spot the Tower of Penance, and as you approach the Vestige bed, a boss fight with Blessed Carrion Knight Sanisho will initiate. Do keep in mind that this boss rises after its health bar reaches zero—you must instantly deal more damage to it so that it falls again.
